Registered Nurse – Homecare
Location: Essex County, NJ
Position Summary
We are currently seeking a dedicated Registered Nurse to join our Homecare team in Essex County, NJ. This role is responsible for providing skilled nursing care to patients, supporting their health and well-being through compassionate, community-based care.

This position serves a community-specific territory within Essex County. Nurses in this role are scheduled within these communities, providing consistency in scheduling and day-to-day responsibilities.

Responsibilities
What You Will Do:
- Assess and evaluate patients with multiple disease processes and comorbidities.
- Develop, implement, and update individualized care plans.
- Document changes in patient conditions and progress toward goals.
- Identify patient and family teaching needs and implement appropriate strategies.
- Monitor the quality and appropriateness of services delivered.
- Collaborate with healthcare providers and interdisciplinary team members.
- Follow agency policies for infection control, safety, and documentation standards.
- RN with Bachelor’s degree in Nursing preferred
- 1+ year of supervised clinical experience as a registered nurse
- Excellent verbal and written communication skills
- Strong computer skills
- Excellent organizational and interpersonal skills
- Car and valid driver’s license
