Community Hospice Admissions Registered Nurse

-Full Time and Part Time-

In this role, the Hospice Admissions RN is responsible for screening and admitting prospective Hospice patients in a home care setting. The ability to conduct a brief physical assessment of the major body systems, obtain the appropriate consents, and establish the initial plan of care for the patient will be expected. As will, thoroughly explaining the Hospice philosophy, available services and advance directives. Electronic Medical Record Experience a must.

Community Registered Nurse

-Full Time-

In this role, the Community RN is responsible for providing nursing care to hospice patients, residents and/or families within their home setting, focusing on symptom management and pain control. The quality of care provided will be consistent with the Standards of Practice determined for the hospice program.

Requirements for both positions include:

  • Current NYS RN License and BLS Certification
  • 1-2 years of prior experience
  • Strong knowledge of hospice concepts, philosophy, policies and procedures
  • Exceptional professional and empathetic communication skills
  • Knowledgeable in the Hospice Medicare benefit, as well as the understanding of other insurances
  • Bachelor’s degree preferred
  • Prior experience in home care, hospice and/or oncology also preferred
